A Year of Collective Performance

2025 was a year of exceptional performance, innovation, and teamwork. We are so grateful to our amazing team of physician organizations, practices, care teams, and patients who make this work possible.
Welcome to MCT2D's 4th Annual Report!
In 2025, the Michigan Collaborative for Type 2 Diabetes delivered results on its first statewide performance measure aimed at improving A1C control. Across 340 participating primary care practices, 558 patients newly achieved A1C control– lowering their risk of serious complications.
Building on this momentum, MCT2D expanded to include an additional performance measure for primary care physicians, as well as initial performance measures for endocrinologists and nephrologists. Primary care physicians and nephrologists are increasing uACR screening to support earlier detection of kidney disease, while endocrinology practices are implementing consistent interpretation of CGM data to ensure that patients prescribed CGM devices achieve optimal clinical outcomes and value. These guiding performance measures build on the foundation we've established as Michigan's first statewide diabetes quality improvement collaborative.
This year, MCT2D led new, innovative programs to drive collaborative quality improvement, including kicking off the development of our prediabetes initiative, launching the first-ever MCT2D low carb counseling workshop, and conducting our first set of site visits to eight practices across Michigan.
Finally, we shared many high-impact tools, including publishing our work on developing the MCT2D Coverage Checker in the Annals of Family Medicine.
This annual report is our 2025 season recap: performance measures launched, partnerships strengthened, and a statewide team proving that when we improve together, progress in type 2 diabetes care is measurable, shared, and built to last.

Tracking the metrics that matter on our path to improving diabetes care for all.
As the collaborative's inaugural performance measure, MCT2D primary care practices achieved measurable improvements last year, with an absolute 2% improvement in Commercial patients achieving an HbA1c <8% and an absolute 1% improvement in Medicare patients achieving an HbA1c ≤9%.
Target rates for the A1C measure have now been updated to reflect 2026 HEDIS 90th percentile rates of 74.45% for commercial patients with an HbA1c <8% and 91% for Medicare patients with an HbA1c ≤9% (Performance measurement period: 7/1/2025 – 4/30/2026).

The collaborative helped over
550 patients
meet their A1C target and reduce their risk for serious complications.
Improved glycemic control can mean fewer medication adjustments, fewer emergency room visits, and greater confidence in self-managing diabetes.
These gains can translate into preserved vision, mobility, and independence, enabling patients to participate more fully in work, family, and community life.

MCT2D expanded its primary care performance measures to include annual uACR screening, a critical component of early kidney disease detection and timely intervention.

In alignment with primary care efforts, MCT2D nephrology practices are also working to increase uACR screening rates.

Continuous glucose monitors provide real-time glucose data that supports both clinical decision-making and patient self-management in type 2 diabetes. To ensure this data is used effectively, MCT2D endocrinology practices are focusing on the download, interpretation, and integration of CGM data into care plans for patients on a CGM.

Building Strength for Prevention
Like off-season conditioning that helps prevent injury and builds endurance, our Prediabetes Initiative will equip patients and providers with tools, education, and resources to prevent progression to diabetes.

The MCT2D resource library isn't just a collection of PDFs; it is the equipment room, where every collaborative "athlete" can go to get the fuel and gear needed to stay at the top of their game.
Our equipment room got a major upgrade with new tools designed to improve diabetes care.

Print tools, including clinic posters, meal plans, and educational key chains, were shared at regional meetings and during in-person site visits. MCT2D participants highly value access to printed tools, noting that many patients are not comfortable with accessing resources online. Printed tools ensure all patients have access to education to address their health.

Participating MCT2D practices could place orders for specific printed resources. Additional resources were distributed at in-person events including the spring regional meeting, collaborative wide meeting, and during site visits.

The spring 2025 regional meetings focused on sharing actionable clinical information that clinicians could take back and implement into their practice.
We were thrilled to welcome our endocrinology and nephrology colleagues alongside our primary care practices for a dynamic meeting filled with relevant updates, expert insights, and new opportunities for collaboration.
I loved the networking first and foremost; secondly, the valuable information learned from endocrinology and nephrology and ability to implement into my everyday practice.
At the fall 2025 regional meetings, high-performing MCT2D practices shared their strategies for success and workflows for urine albumin-to-creatinine screening. MCT2D also shared plans for a prediabetes website for both patients and providers, to be launched in 2026. We also shared clinical updates and went through a detailed review of the learning community requirements.

Our collaborative poster project brought together endocrinology and primary care teams to showcase their innovative partnerships in diabetes management and care coordination. These presentations highlight the power of interdisciplinary collaboration to improve clinical outcomes and share best practices across our network. Collaborations began in April 2024 and outcomes were shared at the Spring 2025 regional meetings.
Each endocrinology practice has created a poster (see sliding gallery below) to showcase the results of their work at each of the seven regional meetings across the state, allowing attendees to learn about these collaborations, including successes and lessons learned.

The primary focus of our collaborative wide meeting this year was supporting practices and physician organizations in achieving the MCT2D performance measures.
At the meeting we:
Voices from our Patient Advisory Board are shaping better care. We consult with the PAB like how you might bring in a veteran athlete to advise you on technique. When we design with patients instead of for them, we create tools that truly make a difference in daily life.
Our Patient Advisory Board brings the most important perspective to everything we do: the lived experience of the people we serve. Their insights drive innovation, improve our tools, and keep us focused on what truly matters.
Felicia is a beloved auntie, a hard rocker, and a person with multiple health conditions, including type 2 diabetes. Felicia kicked off our 2025 Collaborative Wide Meeting with an electric testimony on her health care experiences, navigating the challenges of self-advocacy and multifaceted identity. She brought necessary light to our collaborative wide discussion on health equity and data in quality improvement.
Lynda is a former english teacher from Northern Michigan, proud grandparent, and person with multiple health conditions, including type 2 diabetes, chronic kidney disease, and heart issues. Ms. Lynda kicked off our Spring 2025 regional meetings, testifying to the importance of care coordination, self-advocacy, and her stellar relationship with her primary care physician.

Diabetes Prevention Platform
PAB members have played a vital role in the conceptualization, patient-facing language, and design of our forthcoming patient education platform for prediabetes.
Like coaches developing elite athletes, we invest in knowledge sharing and skill development. Every event is designed to empower our community with the knowledge and skills needed to provide the most current, highly recommended care.
MCT2D hosted two weeklong educational workshops on low carbohydrate diets titled Elevate Your Care: Low Carb Counseling for Healthcare Professionals. This was developed in response to interest from the collaborative in receiving more extensive low carbohydrate education.
Each workshop is structured like a championship training regimen with intensive learning, hands-on practice, recovery breaks, and team collaboration. Participants engage with cutting-edge research, master meal planning strategies, and develop communication skills to effectively counsel patients on sustainable low-carb lifestyle modifications.

Participants used a digital app to track their food and beverage intake for 3 days to reinforce concepts and gain experience needed to teach patients.

MCT2D learning community events are monthly, hour-long virtual sessions led by experts in their field providing education on topics that the collaborative requests. Learning Community Events are open to everyone and are a great resource on T2D related topics.
Top athletes know to test their gear before competition. The CGM User Experience Program connects MCT2D clinicians with CGM devices, providing hands-on experience and valuable insights into how CGM can transform diabetes management. Like athletes testing new equipment before competition, participants explore CGM features, learn about the patient experience, and help shape future tool development based on real-world use.

Since its inception, over 250 collaborative members have participated, providing invaluable feedback that shapes CGM education and implementation strategies.

In response to our members' requests for an "easy button," the Michigan Collaborative for Type 2 Diabetes (MCT2D) developed the MCT2D Coverage Checker. This web-based tool provides preferred coverage, step therapy, and PA guidelines for guideline-directed medical therapy (incretin mimetics and sodium-glucose cotransporter 2 [SGLT2] inhibitors) and continuous glucose monitoring (CGM) devices for type 2 diabetes (T2D) for specific plans with a few easy clicks.
