ENGAGEMENT REQUIREMENTS

As a member of MCT2D you are required to complete certain activities to receive your value-based reimbursement (VBR)

LEARNING COMMUNITY REQUIREMENTS

(Engagement VBR)

PHYSICIAN LEVEL

PICK ONE: Every primary care physician participating in MCT2D must complete ONE of these activities by June 1, 2024.

Attend a learning community event or watch a recorded event and answer a brief quiz

Test (pilot) a Learning Community tool and give feedback on its use in a clinical setting.

PRACTICE LEVEL

PICK ONE: Each practice must complete ONE of these activities by June 1, 2024.(Can be completed by any member of a participating practice)

Gather feedback from a patient on a Learning Community tool.

Participate in a Practice-To-Practice (P2P) Best Practice discovery session.

Share a dotphrase to our MCT2D Dotphrase Library and participate in a 15-min lightning discussion about your phrases with our team.

PHYSICIANS ORGANIZATION LEVEL

PICK ONE: Each PO must complete ONE of these activities by June 1, 2024.(Can be completed by any member of a participating practice)

Contribute your expertise through an interview for a blog or video with our coordinating center team.

Share your expertise at a live event as a panelist.

Refer a patient to share a story, quote, or other feedback about their T2D care.

Activity: Provide a short, virtual interview for our collaborative blog or video series, like pharmacist and clinical champion Heidi Diez’s insights into Patient Assistance Programs for medications.

Impact: Sharing your expertise, big ideas, or lessons learned can have a ripple effect for the 800+ members of the Collaborative who are working on the shared goal of delivering the best available care to your patients and communities.

Time commitment: 1 hour, online

The Basics of Team-Based Care Billing and Coding

March 3, 2023
In this joint presentation, Ashley Schwartz, LMSW, of the Michigan Institute for Care Management and Transformation, shares the foundations of team-based care billing, focusing on reimbursement for care management services for BCBSM and Priority Health. Ashley will walk viewers through examples of applying care management billing codes to clinical practice, point out opportunities to optimizing documentation and billing, and ground the discussion in building sustainability into team-based care. Ms. Schwartz's talk is modeled after MICMT's Foundational Care Management Codes & Billing Opportunities online course.
In the second half of the discussion, Lauren Oshman, MD, MPH, will share the codes you need to know for non-face-to-face type 2 diabetes care, with in-depth case examples, documentation requirements, and RVU in context.

Management of CKD: A New Era of Therapeutics

February 13, 2023
In this presentation, Dr. Mike Heung begins by describing the initial evaluation of chronic kidney disease, including the origin of the new eGFR equation and importance of albuminuria testing. He goes on to describe recent advances in therapeutic options for patients with chronic kidney disease, including SGLT2is and MRAs.

Clinical Application of 2022 Updated T2D Guidelines from the ADA, EASD, and the AACE

November 2, 2022
In late September 2022, three key professional organizations published updated guidelines for type 2 diabetes. The American Diabetes Association (ADA) and the European Association for the Study of Diabetes (EASD) released a consensus report, "Management of Hyperglycemia in Type 2 Diabetes," and the American Association of Clinical Endocrinology published, "Clinical Practice Guideline: Developing a Diabetes Mellitus Comprehensive Care Plan—2022 Update."
In early November, we held a live webinar presentation to engage with the updated guidelines, with a featured presentation from Dr. Jonathan Gabison, MCT2D content expert, obesity specialist, and family physician.

CGM Operationalization Panel Discussion

September 1, 2022
We invited a live panel of MCT2D members with expertise in implementing continuous glucose monitors (CGMs) in their primary care practices.
The panel discussed the following topics:
  • Identifying patients who may be right for a CGM.
  • Workflows associated with receiving CGM data from patients.
  • Workflows associated with how CGM data is uploaded to the EMR/reviewed prior to, or during patient appointments.
  • Roles in the office related to CGM prescribing (e.g. introducing a CGM to patients, providing education, uploading the data to the EMG, reviewing the data, etc.).
  • Strategies for integrating CGM prescribing into a practice.
  • Navigating insurance coverage for patients with Type 2 Diabetes.
