Download and distribute a one-page recruitment flyer for our Patient Survey. The Patient Reported Outcomes Survey is a paid opportunity for patients to provide feedback on their health and health care experience through three surveys distributed by text message, email, or mail over the course of 12 months. Patients can complete the PROS here.
