Michigan Collaborative for Type 2 Diabetes is a coalition of over 1000 physicians, coming together from across 346 primary care, endocrinology, and nephrology practices, all of whom are represented by 28 physician organizations spanning the state.
Each practice unit elects at least one clinical champion to represent the practice in specialized trainings, committees, regional, and collaborative-wide gatherings. Here’s a breakdown of the 2021 MCT2D Clinical Champions, by profession.
