This five-minute video introduces MCT2D to new practices joining the collaborative
The Michigan Collaborative for Type 2 Diabetes (MCT2D) welcomes all 360 new and returning member practices to the 2024-2025 Value Based Reimbursement (VBR) year. Our network of primary care, nephrology, and endocrinology practices share a commitment to the prevention of type 2 diabetes and its complications. Together, we are creating a vibrant community of clinicians and patients to accelerate the equitable implementation of evidence-based diabetes care for all patients in Michigan.
September 1st marks the start of a new VBR year. On this page, we hope to orient new practices and outline key updates to value-based reimbursement operations in this 2024-2025 cycle.

New MCT2D Primary Care Practices
Welcome to MCT2D! All new practice clinical champions will have received a welcome email letting you know about the next steps. This first year in MCT2D is all about learning where you and your practice currently are with type 2 diabetes and introducing you to the MCT2D initiatives:
Guideline-directed medication prescribing
Promoting the use of continuous glucose monitors (CGM)
Promoting low carbohydrate eating patterns in patients with type 2 diabetes
Your onboarding to the program involves three different levels of assessment:
The practice change readiness assessment. This was described in the email you received and aims to provide an understanding of your practice structure, comfort level with quality improvement, and readiness to tackle our three main initiatives.
Physician survey. We ask that each physician in your practice complete a brief, 11-question survey about their specific familiarity and comfort with the approaches outlined by MCT2D's core initiatives.
Patient experience survey. We are asking that each practice collect ten patient-focused surveys through December so that we can understand the patient’s experience of their type 2 diabetes care.
You’ll receive emails throughout the month of September providing more details on each of the above activities.
Clinical Champion Training
Above is a brief video introducing the clinical champion role.
A clinical champion from each practice will also complete training on each of our quality initiatives. Trainings will take place live one time at the dates and times below, and then be offered virtually on demand. CME will be provided for both the live and recorded versions of CME.

Returning MCT2D Practices
There are three new aspects to the 2024-2025 VBR year that returning practices should note:

1. MCT2D Performance Measure
In this cycle, MCT2D is introducing a performance measure. Participation measures will count for 3% of your VBR, and the performance measure will count for 2%.
The initial MCT2D performance measure is based on uncontrolled A1C values. The percent of members age 18+ with type 2 diabetes whose most recent glycemic status (A1C) was at the following level during the measurement year. MCT2D will be aligning with the HEDIS 90th percentile rate for commercial and projected 90th percentile rate for Medicare populations:
For practices already meeting the metric, you will need to additionally complete one of the following options to earn your VBR:
Chart review of 10 patients with an A1C over 9%
Participation in a workgroup around submitting a Glucose Management Indicator (GMI)
Practice Size
If your practice had 100 or more patients with an A1c within the time frame of 6/1/2023 - 5/31/2024, then your practice’s performance will be scored as an individual practice.
If your practice had less than 100 patients with an A1c during that time frame, your practice will be counted with other practices in the PO with less than 100 patients.
2. Practice Level Scorecard
We are introducing practice-level scorecards as a new way to help you track your performance on MCT2D VBR measures. The scorecard format includes all of the requirements that your practice must meet to earn the VBR. The practice-level requirements will be familiar to you, but the scorecard format is a new way to show you clear and actionable information at a glance.
This year, we have assigned points to each of the requirements totaling 100. In order for the physicians in the practice to earn VBR, your practice must earn a minimum of 80 points. An example is shown to the right.
Note: Participation measures will be similar to what you have seen in previous years- attendance at the regional meetings and meeting the physician, practice, and PO level learning community requirements. Additional options to meet these learning community requirements may be added throughout the VBR year.
How to Access the Admin Portal
To track your practice’s completion of the VBR requirements, please sign up for an account on the MCT2D Admin Portal. To do this, go to portal.mct2d.org and click “Apply for An Account” along the left hand side. Once your PO approves your application, you will be sent a link to reset your password.
3. VBR End-of-Cycle Deadline
One major change is that you have an additional month to meet your learning community requirements. BCBSM has extended the deadline for VBR requirements through July 1st, 2025 instead of June 1st as it has been in previous years.
