Practices must achieve a 5% improvement in CGM interpretation rates, up to the 95th percentile benchmark (89.59%). The document includes eligibility, denominator/numerator definitions, baseline and intervention periods, reporting schedule, and VBR scoring framework to strengthen CGM use and patient care.
