In August, the MCT2D Learning Community webinar featured guest speakers Sarah Kile, Director of Community and Partnership Engagement, from Michigan 211 and Janna Foster, Rn, BSN, RN Care Manager for MyMichigan-Frankenmuth, with MCT2D Design Project Manager Larrea Young, MDes.
Kile presented on Michigan 211, a shared social services resource with regional centers to support individuals and households across the state. Foster represented featured MCT2D practice, MyMichigan-Frankenmuth and her practice's approach to screening for non-medical factors that influence health.
Young, MCT2D design project manager, shared updates on care for all initiatives within MCT2D.
