As a patient, it can be really hard to figure out what’s covered, what’s not covered, how much you’ll have to pay out of pocket, and what drugs might be more affordable and as effective as others that are more expensive.
This fillable or printable worksheet will walk you through what to ask your insurance company after you have been prescribed a new diabetes medication, like a GLP-1 RA or SGLT2i, Continuous Glucose Monitor (CGM), or traditional glucometer.
