All primary care clinical champions must complete the series as part of onboarding as new members of the Michigan Collaborative for Type 2 Diabetes (MCT2D).
Objectives
  1. Enhance the management of T2D by promoting best practices in diabetes care
  2. Reduce complications related to diabetes, such as cardiovascular disease, kidney failure, and neuropathy
  3. Improve clinical outcomes by providing support to healthcare providers across Michigan
