VBR

• Final participation measure scores for the PCP VBR are available in the portal
• POs who have had a practice who has not met the A1c control measure have been notified. Final reports will be available on the Admin Portal the week of 7/14.
• Following the distribution of the PGIP list from BCBSM, we will share a list of physicians and their VBR score in the portal for you to review and sign off on for the upcoming 9/1/2025 VBR year.

Performance Measures

• The performance measurement period for the PCP uACR measure, the nephrology uACR measure, and the endocrinology CGM interpretation measure began on 7/1/2025. The measurement period for all three measures will go through 8/31/2026.
  • PCP uACR baseline rates are available in the Admin Portal.
  • Nephrology uACR baseline rates were distributed via email to clinical champions and PO leads on 6/2/2025
  • Endocrinology CGM interpretation baseline rates will be distribute the week of 7/14.
• The PCP A1c measurement period began on 6/1/2025, following the same timeline as the previous year’s A1c measure, with the measurement time frame ending on 4/30/2026.
  • Baseline reports will be distributed in August for the A1c measure.

Endocrinology

• Please note that the endocrinology CGM interpretation survey is due on 8/1. One survey needs to be submitted per practice and this can be found on the Admin Portal.
