The Michigan Collaborative for Type 2 Diabetes (MCT2D) is excited to welcome our 351 primary care practices (PCPs) to the 2025-2026 PCP Value Based Reimbursement (VBR) year.

Primary Care (PCP) Performance Measure Overview


With the launch of the new PCP value-based reimbursement cycle on September 1, we’re excited to begin another year of meaningful work together.
MCT2D is focusing on two primary care performance measures this 2025-2026 VBR year: increasing urine albumin creatinine screening rates and HbA1c control. Our June 2025 newsletter focused on the new uACR measure, providing measure details and giving an overview of the goal rate.
  • Work toward a 10% absolute improvement in screening rates from the practice's baseline, up to a maximum goal of 90%.
  • : Continuing for a second year. Final goal rates will be shared in November after updated HEDIS 90th percentile benchmarks are released.
  • PCP Reporting: Updated uACR and HbA1c reports (through 8/31/25) will be posted to the MCT2D Admin Portal in November.
    • uACR reports: Nov 2025, Mar 2026, Aug 2026, Final: Feb 2027
    • HbA1c reports: Nov 2025, Feb 2026, Final: Jul 2026

Simpler Learning Community Engagement for Primary Care


We’ve refreshed our Learning Community to make it easier to participate and earn credit, while staying aligned with your workflows and our shared goals and measures. Options include:
  • Primary Care Physician level
    : Live webinars, CGM trial, recorded sessions + quiz, tool feedback.
  • Primary Care Practice level:
    Portal/website feedback, focus groups, motivational interviewing coaching/simulation, uACR provider facing tool feedback survey.
We’ll review all of these options in detail at the upcoming fall regional meetings. Our goal with these updates is simple: to make participation easier and more valuable, while providing meaningful support for advancing type 2 diabetes care. As a reminder, any practice who has their physician level and practice level learning community requirements complete by 1/1/2026 will earn 5 points extra credit on their VBR scorecard. Specialty Care Program (SCP) Performance Measure Overview

Measurement period: 7/1/2025–8/31/2026

Nephrology uACR Performance Measure

  • Focus: Ensure patients receive appropriate screening in order to detect early kidney disease.
  • Goal: Work toward a 5% absolute improvement in screening rates from the practice's baseline, up to a maximum goal of 90%.

Endocrinology CGM Interpretation Performance Measure

  • Focus: Ensure patients on CGM receive interpretation and guidance (CPT 95251).
  • Goal: Increase the percentage of CGM patients with 95251 billed by 5%, from the practice's baseline, up to a maximum goal of 89.59% (95th percentile for collaborative).
