- This database was created using each participating practice’s submission of the Best Practices Survey.
- Using the filters below, identify sites that are similar to yours so that the best practices shown are based on sites with similar resources, patients, and practice demographics.
- All filters are optional, and results will appear below as you add and remove filters.
- If you are getting too many results, add additional filters. If there are too few results, remove some of the filters and additional results will be displayed.
- You can also search for a specific word or phrase, such as “CGM” or “prior auth” with or without using filters.
- When results are displayed, you can click “Show Demographics” to view/hide specific site information in addition to the filters that you set.
- Additionally, sites that agreed to be contacted about their submission will have a button that says, “Contact Practice.”
- If you wish to learn more about how they specifically implemented this best practice or have additional questions that weren’t answered as part of the submission, we encourage you to reach out to the site.
- If you click this button, you will be asked to enter information about yourself, and an email will be sent to our MCT2D Coordinating Center team.
- Staff at the coordinating center will then put you in touch with the site who submitted the best practice.
- If you connect with the submitting site and have a discussion, this will meet your practice level learning community requirement.
